What’s new with Sculptra

Sculptra is not a filler- but a product that is injected into our skin (generally on the face) to stimulate collagen production. It consists of PLLA, which is the ingredient found in many of our absorbable sutures. This is a product that has been around for a long time and is well tolerated by our bodies. When collagen is stimulated, it adds volume to the undersurface of our skin and also makes our overlying skin appear younger and fresher because of a better supporting structure. This is often referred to as a Sculptra “glow”. The extra volume lessens folds and wrinkles to create a more even, level surface. Sculptra has also been used “off label” to improve skin on the body, such as on the chest or buttocks.

One of Sculptra`s benefits is that it lasts longer than most fillers. Typically, a patient needs 1 vial (bottle) per decade of life. So, a patient in their 50s would need five bottles given at monthly intervals. Most treatments start with 2 vitals to accelerate results. An annual touch up vial is recommended thereafter for maintenance.

So…what’s new??? In the past it was felt that Sculptra needed to be reconstituted 48 hours ahead of an appointment. We now know that it can be injected immediately at a visit; with preparation at the time of the visit. There is no need to postpone treatment or to call 2 days in advance for an appointment. We also won’t take a deposit; since it isn’t being prepared for you prior to coming in.

Dangers of Benzoyl Peroxide

A few summers ago, we worried about spray sunblocks containing the cancer causing chemical: Benzene.

This week we learned that this ingredient is found in many over the counter benzoyl peroxide acne products. Brands tested and found with dangerously high levels include Clinique, Target’s Up & Up, Proactiv, PanOxyl, Walgreen’s acne soap bar, and Walmart’s Equate Beauty. FDA guidelines allow up to 2 parts per million (ppm). Valisure Testing Laboratory found up to nine times that amount in some treatments. These levels jumped significantly when the products were tested at higher temperatures as if stored in a medicine cabinet in a steamy bathroom. Proactiv’s 2.5% benzoyl peroxide cream contained as much as 1,761 ppm of benzene during testing; A similar cream from Target reached 1,598 ppm; Clinique’s hit 401 ppm. Clearasil 10% BPO initially tested just at the FDA level but jumped to 308 ppm.

The benzene found in sunscreen was an impurity that came from contaminated ingredients. The benzene in BPO comes from the benzoyl peroxide itself. BPO can break down and form benzene.

Valisure’s testing also examined benzene in the air surrounding acne treatments. An unopened Proactiv product leaked high levels when kept at 104 degrees F; the temperature of a hot shower.

When testing was done with salicylic acid (another acne ingredient found in OTC products) elevated benzene was not found.

So…what to do? I don’t recommend BPO for acne, but for an entirely different reason. Although an excellent treatment, these products can permanently bleach fabric. I don’t want someone to ruin their clothing, bedding, towels, etc. We have many wonderful topical treatments for acne by prescription. The Retin A family is always expanding, we have topical tazarotene, topical antibiotics, Dapsone gel, etc. We have excellent oral medication to treat (and cure) acne and many office treatments to use as adjuncts.

Skin Cancer News

Jill Biden went for Moh’s microsurgery. What is that? It is an outpatient type of skin cancer surgery. It is done under local anesthesia in a facility where there is an operating room and laboratory for simultaneous examination of all skin removed. If a section is positive for skin cancer then additional tissue is taken until clear borders are reached. This technique ensures full removal of certain skin cancers (generally basal cell and squamous cell carcinomata of the face) with a minimum scar.

Typically a skin biopsy is done first, and you would only go for Moh's microsurgery if skin cancer is diagnosed.

As part of your new year’s resolutions you should remember to get a full body check for skin cancer. Many patients have skipped this needed annual exam during covid and are coming in with many more skin cancers than typical. In between checkups if you spot anything crusting, bleeding, or changing color you should call the office to come get it examined right away. If neglected, it is more complicated to treat.

Daily wearing of SPF 30+ is also recommended on all skin exposed to sun. This means the face and in warmer weather the neck, chest, arms, back, and legs. Too many patients still think that sunblock is just for the beach.

1. Oral Minoxidil for treatment of hair loss. Topical minoxidil was discovered accidentally when a patient on high dose oral medicine grew hair on his head ! Now we are back to using this oral medication – but in a much lower dose – to regrow scalp hair. I have used it in both men and women with excellent results. Blood pressure is unaffected at this low dose and you see results all over the scalp. How wonderful to have another drug for treatment of hair loss.

2.New drugs to treat Vitiligo, Alopecia Areata, Psoriasis, and Atopic Dermatitis. My accountant wonders why every time he turns on the TV there is a commercial for a dermatology drug. The JAK inhibitors, both topical and oral have taken over dermatology.

3.Approval of Daxxify, the longer lasting Botulinum toxin. Your treatment will last for 6 months. Better than a gel manicure. This should be available for our use early in 2023.

What's new in Botox???

1)The latest trend in Botox Is to use less medication; meaning lower doses and lower prices. This creates a more natural look and still relaxes the expression lines we so hate.

Alle, the maker of Botox has implemented a new suggested dosing. Sixty-four units is the suggested full face treatment dose. A full vial of Botox contains 100 units. For older patients, with more extensive and deeper lines, dosing closer to 100 units may still be necessary between treatment and touch up. For younger patients, whose lines aren’t as “etched in” the lower total dose may be sufficient to treat and prevent progression to deeper lines. The three facial areas (frown, crow’s feet, and forehead) can all be treated with the 64 unit dose.

2)We now have FDA approval of Daxxify; the longer lasting (six to nine months) botulinum toxin treatment. Approval for new drugs was held up during covid, but those in the know have been waiting patiently for this to become available. Results are seen as early as one or two days after treatment and typical duration of treatment was six to seven months. No adverse effects were reported. We will let you know when this will be available for you to have.

How many sunblocks do I need?

Answer: more than 1

The best sunblock is one that you will use regularly. Dermatologists recommend daily use of SPF 30+ on all skin not covered by clothing or your mask. It should be applied on a year round basis; not just when sitting by the pool or when on a beach or a boat. On days when you are near that is water resistant, so that with excess sweating/swimming/reflection it remains on your skin.

For faces I do not recommend a spray. There’s too much risk of danger to your eyes if the spray enters. A lotion or stick is best for the face. A stick works exceptionally well for athletic individuals, since it won’t move with sweating, and is great for lips. (Think Herpes prevention.) Choose an oil free option if you are acne prone. A new product from Elta MD is a touch up balm; UV Stick Broad spectrum SPR 50+. This product can be reapplied throughout the day without disturbing your makeup and fits in a purse! This is the 2022 equivalent of repeatedly “powdering your nose”.

To add to sun protection of your face, a wide brimmed hat works well. A baseball cap is stylish- but only protects the top of your forehead- which is not sufficient.

For the body I do love an SPF spray. The new cans spray in any direction and the sunblocks feel lightweight on the skin, even when waterproof. You can spray your neck, arms, legs, back, etc. quickly and efficiently. I know there was the Benzene scare last summer. The sunblocks for sale in 2022 are all Benzene free.

Many facial sunblocks contain anti-aging peptides, niacinamide to reduce redness, antioxidants, hyaluronic acid, and other ingredients which enable them to multitask. Pick a tinted one and let it become your makeup!

What's New in Hair Loss?

First, I am quoted in the May, 2022 issue of Elle Magazine. The article is on hair shedding- and lists many causes for it. Covid 19 infection should also be included. The take home message is that if you are losing massive amounts of hair, it could be more than a cosmetic concern. You should make an appointment to come in for a visit to search for the hidden medical cause, (think thyroid problems, anemias, etc.) which will prevent your hair from growing back unless identified and treated.

I will also be quoted in an article in the July/August 2022 issue of Good Housekeeping magazine. I haven’t seen the article yet- but it is also on hair loss.

There is also much media attention to a newly approved drug for treatment of Alopecia Areata; an autoimmune cause of hair loss Unlike hair shedding, hair loss in this condition is patchy; often with large areas of shedding in one area. Bald areas can be coin shaped or larger, and can even involve the entire scalp. (This is probably what Jada Pinckett has when she talks about having “alopecia”.) Until now treatment has consisted of injecting cortisone into the affected areas on a monthly basis until the hair regrows. When the area is significantly large this becomes difficult. I have treated people with the home laser (Theradome helmet) with good results to supplement office injections. The newly approved medicine is a JAK inhibitor and is taken orally. Due to side effects this medication is generally reserved for patients with loss of 50% or more of their scalp hair.

We are also implementing the second generation of PRP injections for treatment of hair loss. Come in for a complementary PRP consultation to find out how this works. There will still be blood drawn during your visit and reinjected back into your scalp . The technology is now different and a gel matrix ensures that the growth factors from your blood remain in your scalp for at least 1 full week. Previously the factors were absorbed much more quickly. Results have been excellent’ especially for androgen caused hair loss in both men and women; but PRP works to treat all types of hair loss.

It’s Totally Okay To Shed

Seeing a clump of hair swirling down the drain can be scary. But it turns out daily hair loss is normal and rarely something to stress over.

I’ve been a champion hair shedder for as long as I can remember. A small girl with a disproportionately large mop of hair, I recall repeatedly breaking my mom’s fine-tooth combs and meekly asking my dad if he could snake the drain. Again. And while stylists would often tell me how lucky I was to have such thick hair , I often felt anything but.

My shedding only seemed to intensify when I became an adult. It is now pretty much constant and incessant. Whenever I brush my hair or blow it dry with my Dyson, I’m greeted with a fistful of my own tresses. Countless roommates—and a few significant others—have been moved to comment on my abundance of strays, impossible to miss as they’re strewn across the bathroom floor.

There’s a reason some men, in particular, are startled by the volume of strand shedding. For one, women tend to have longer hair than men, and if you have longer strands, “it can feel like there’s more shedding happening because the hair ball looks bigger,” says Ronda S. Farah, MD, a dermatologist based in Minneapolis. New York City dermatologist Joyce Davis, MD, concurs: “Circling in the drain, one long strand of a woman’s hair is going to look like more than three short hairs from a guy’s scalp.” (It should be noted that there are a number of men who prefer that their SOs have long hair, so why judge them for shedding it? Hypocrisy at its finest.)

Yet I’m assured shedding is totally normal. And because my hair is so thick and grows quickly, the loss is routinely replaced, Davis tells me. “Young women with very thick hair have the metabolism to keep up with [the shedding],” she says. “They make hair, they lose hair; they get ahead of it.”

According to Davis, a typical amount of daily shedding is up to 100 hairs. “If you’re losing closer to 300 hairs a day, you should probably get it checked,” she cautions. Although that’s not to say you should count every last strand obsessively. “Be reasonable about it: See what’s in your brush; see what’s on your clothing; see what’s on your pillowcase; see what’s in the shower.”

If you do note a slight uptick in shedding, it’s typically not cause for alarm, says Megan Robinson, owner of Pearlita , a salon in New York City’s Nolita neighborhood. She believes increased shedding can be seasonal and often escalates in the summer. (There’s also evidence that hair may grow faster during warmer months, presumably because heat boosts blood flow to the scalp. So, in theory, new growth would keep pace with summer shedding.)

Another common cause of heightened shedding is an imbalanced diet, particularly a protein deficiency. “Hair is protein, but it’s the least important protein your body makes,” Davis says. So if you’re not eating enough protein, she explains, hair gets short shrift because your body prioritizes using protein reserves to maintain vital functions—not to nourish hair. She recommends taking a multivitamin with iron and eating at least two sources of amino-acid-rich protein daily—ideally from an animal source—to ensure that a nutritional deficit isn’t the root cause (pun intended) of increased shedding.

Other conditions that can intensify shedding: anemia, thyroid abnormalities, hormone swings (post-pregnancy or during menopause), certain medications, and overstyling that causes hair to break off or follicles to release strands en masse. (The latter is called traction alopecia and can be the result of wearing very tight styles, such as braids, for extended periods of time.) Illness, surgery, or a stressful life event can trigger a condition called telogen effluvium, in which up to 50 percent of the hair prematurely enters the shedding phase. Fallout from telogen effluvium typically occurs two to three months after the diagnosis or event in question, and the increased shedding may last for up to six months, says Cleveland dermatologist Shilpi Khetarpal, MD.

The good news? These conditions can all be addressed, and most are temporary. A doctor can prescribe treatment for an underlying condition, but there’s a lot you can do at home to boost new growth. Dermatologists frequently recommend minoxidil, such as Hers Minoxidil 2% Topical Solution , an over-the-counter drug applied topically to the scalp to increase follicle size and extend the growth phase. There are also a variety of drug-free scalp treatments that nourish the skin and boost blood flow to follicles, contributing to overall healthier hair. Try Nutrafol Growth Activator , Augustinus Bader The Scalp Treatment , or Virtue Flourish Density Booster .

Meanwhile, keep the shower (and the drain) cleaner with this easy trick from Robinson: “Brush your hair with a mixed-bristle brush before you shower,” she says, recommending the Crown Affair The Brush No. 001 . “That way, all the hair that would otherwise shed into the drain comes out on the brush instead, keeping your shower (and your partner) much happier.”

And the next time you discover a tangle of strands on your pillow or bathroom floor and feel like freaking out? Remember what Farah said to me: “It’s normal to shed hair. Everybody is shedding hair all the time, and it can vary day today.” So take a deep breath, toss it in the trash, and carry on.

Yes, it's treatable!

More than half of women over age 50 will experience hair loss according to a study published this month in Menopause. Although hair loss is widespread and accepted in men, not much has been published about it in women. What is exciting is that there are new treatments for this problem, which is depressing for many women.

When women present with hair loss it is complicated; since there are different patterns of loss and many reasons for this. A consultation with a dermatologist interested in treating hair loss is the first step. A thorough history is obtained to check for preceding events such as recent surgeries, dietary changes, etc. which could account for the hair loss. There are also hidden medical causes of hair loss, such as various types of anemias, autoimmune diseases, thyroid problems; etc. I typically order a panel of blood tests for my female hair loss patients to check for these (and other) medical problems so that they are not overlooked. Correction of an underlying problem usually stops the hair loss and hair regrows.

We have a few new and exciting ways to treat hair loss; no matter what the cause.

Home lasers are effective, lightweight, wireless, and comfortable. They resemble a bicycle helmet. You would wear it for at least 20 minutes twice weekly and it has been effective in stimulating hair follicles to regrow hair. We have one on display in out waiting room for you to see.

PRP injections into the scalp are also effective for regrowth of scalp hair. I was one of the first dermatologists in Manhattan to offer these all-natural treatments; which use growth factors harvested from your own blood. This is an office- based procedure. I draw your blood and spin it down in a centrifuge. I then separate out the platelets and reinject the growth factors into areas of thinning on your scalp. Several treatments are needed. Sunblock Sprays

Are they safe???

This year there is concern over benzene in our sunscreens. Benzene is a liquid component that is colorless and flammable. It has a sweet, gasoline-like aroma. But, breathing it can cause serious complications if it is present in a high enough concentration.

On July 15, 2021 Johnson & Johnson announced a recall of most of their Neutrogena and Aveeno spray sunscreens from US stores; after detecting benzene, a potentially cancer causing chemical in some samples. Although not added in the manufacturing of the spray sunscreens, it seems to be present as a contaminant. Their Ultra Sheer Weightless Sunscreen Spray SPF 100+, and Ultra Sheer Weightless Sunscreen Sray SPF 70 were among 14 products Valisure claims have some of the highest levels of benzene tested.

Benzene was found in 78 of nearly 300 sprays and lotions tested, including products sold by Aveeno, Banana Boat, CVS, and Neutrogena.

Are there safe spf sprays and lotions? Yes.

In our office we dispense the ELTA MD sunblocks. These are high quality, lightweight, chemical free (except for the aerosol) sunblocks formulated for dermatology offices. They are safe for adults and children. My children used the Elta MD Aerosal SPF 45 during a winter vacation in Barbados and didn’t burn, even though they are very fair.

Elta MD SPF products had low levels of benzene. Valisure reported finding a 0.11 to 0.17 parts per million of benzene in the 5 Elta MD SPF products tested; Including the Aerosal SPF 45. This level is more than 11 times below the recognized international safe guidance adopted by the US FDA for benzene as a residual solvent in drug products.

This month we are thinking about spring. Coming out of winter gloom; dark and cold days and nights. Winter kept indoors by Covid.

Now that most adults have been vaccinated, or have made plans to get vaccinated, we can think of positive happenings.

I look forward to burying my sweaters until next winter and taking out my colorful spring clothes.

Some dermatology updates are a change in PRP technique; a way to regrow scalp hair. I no longer perform microneedling; which people found the most painful part of the procecdure. We thought it was necessary when I started doing this treatment several years ago, but now know that it isn’t true. I can double spin the tube (taking 20 minutes now) and can concentrate the platelets to a higher degree. Results have improved. We also recommend use of the Theradome laser helmet to augment results.

There is a new lip filler on the market; Restylane Kysse. This competes with Juvederm Volbella. Make sure that you sign up for rewards with both Galderma’s Aspire rewards and Allergan’s Alle programs so that you can get extra discounts when you come in for treatments.

New life... new hopes

I know we never thought this pandemic would last so long. But, hang in there. Vaccines are here and hopefully by the fall life can resume. I received my first vaccine 3 weeks ago. I went to Mount Sinai Beth Israel, my teaching hospital since 1983. I will get my second dose this week. Hopefully they will still have supplies.

I took my 91 (almost 92) year old mother to Bellevue for her vaccine. She considers herself very fortunate that she was able to get the vaccine before NYC stopped vaccinating seniors due to short supplies. The scene was like an army MASH unit. Spread out, echoing, loud, but efficient. She received her appointment for dose #2 while there; which other sites (like East Harlem) didn’t offer. She heard from friends in Florida that there, you can get vaccinated at the drugstore in the Publix supermarkets. So different from NY. We couldn’t even get an appointment in Long Island, where she lives. Nassau County was all booked up.

Meantime, I am reading books on my kindle. I like the Baldacci novels. I am baking cookies with my daughter. We found a chocolate chip cookie recipe in the New York Times, which we have made four times already. She explained to a friend that there are always 14 cookies. Although my 15 year old son largely spends his time playing video games; my daughter is busy playing guitar (acoustic and electric) and learning pieces on the piano. My husband, a heart surgeon, works at a hospital; so we both leave each morning. This is a strange year. My daughter’s friend told me which high school she now attends; but admits she has never been there. Let’s hope for the return of normalcy.

Last night we watched Wonder Woman 1984. I really enjoyed the act of watching a movie ! My husband is talking of getting a larger, perhaps 85” set for our weekend house. Certainly we can’t go that big in NYC.

New Year!

Time for New Year’s Resolutions!

What a terrible year we have been through…and the pandemic is still continuing. At least we can look forward to getting a Coronavirus vaccine. I still haven’t received one since I am not hospital based/taking care of Covid patients. But, hopefully l can get one soon.

Mostly, 2020 has been a year of work arounds for school, employment, and entertainment. I am fortunate that I get to leave my apartment each day. I see other people and come home with interesting experiences. Not everyone is.

So, what will my 2021 resolutions be? First, I will work on improving my posture, making sure I sit up and stand up straight. We all do so much computer work, that slouching over a desk/table, has become the norm. I will suck in my stomach as a reminder.

Resolution number two is to try to go back to walking a mile each day. I used to do this before my children were born; but stopped since I felt it was taking time away from being with them. I need to figure out how to integrate this into my routine. Going to the gym seems to formal (undress, change, workout, shower, redress, etc) so just getting out of my taxi earlier to get some walking in may be more practical. I do wear comfortable shoes, so I should be able to get the walking in most days. Of course, the weather has to cooperate.

Last, will be to remind my patients to wear SPF each day. It’s so important. We need SPF of at least 30 on all skin not covered by clothing or a mask. There are so many lightweight sunblocks now, that there is no excuse to skip this step.

Happy Holidays! We need a little happiness after this dreadful year! No dining in restaurants, no movies released in theaters, no Broadway shows, no concerts, restricted sports and gyms, no vacations, travel, etc. No holiday dinners, no parties, no New Year’s Eve celebrations…How depressing ! Nothing to spend money on. Almost.

So, enter retail therapy. What a great term. Making yourself happy with purchases. Look good, feel good. Spend on clothing, jewelry, cosmetic treatments, athletic wear. My apartment building has been getting way more packages than in previous years. I did all my holiday shopping on line this year. I also chose stores where returns are easy (via my doorman/package room) or else at stores near where I live. My wardrobe has shifted away from formal wear to more casual wear. Normally I get a new gown for my husband’s hospital gala each year. Canceled. I now need more sneakers, jeans, sweaters, “weekend clothes” even during the week. Low healed shoes and boots instead of pumps. I still dress to go to work, but now am in the office just three days/ week since Covid.

I bought Jerry Seinfeld’s new book for the men in my life. It is a delight. This type of book works better in paper than on a Kindle, since you want to pick it up and read sections a little at a time to laugh. I saw Jerry when he performed at the Beacon Theatre, my husband’s Hanukkah present for me that year. This year he bought us tickets for Hugh Jackman (one of my favorite showmen) in The Music Man. He has been receiving updates of the regular postponements. Sutton Foster is also sensational. I hope they are available to do the show when it opens.

I bought clothing for everyone. My kids each grew about 3 inches this year and needed longer pants and tops. Not everything was liked. But, I expect that. I also bought an electric mixer for my daughter to have when we bake cookies. Being such a non-cook, I didn’t realize that I didn’t have one.

Growing taller means that hormones are in full gear. I am starting to see acne of my daughter’s beautiful face and my son’s back. Fortunately I have access to all the latest medications for this. I even know that in the pipeline is an acne drug in an entirely new category. It is a topical anti-androgen. Hopefully it will reverse acne that is hormonal. Not just in teenagers, but in menstruating women, menopausal women, etc. This drug should arrive next year. It will help my patients as well as my children.

Wednesday November 18 is the second annual National Botox Day!

What does Botox do? It blocks release of acetylcholine, a chemical in our body. Botox acts as a muscle relaxer where it is injected. It started out as a treatment for strabismus (cross-eyes) in children. It was a non surgical breakthrough. It has been used for treatment of all kinds of neurological/muscle/spasticity problems throughout the body. It treats bladder problems, facial asymmetries, arm and leg contractions etc.

In dermatology it is used primarily on the face. It can eliminate forehead lines, squint lines, and frown lines. So now what?!?!?

Clarisonic is no longer being made. This was a motorized, facial cleansing brush which was “all the rage” for several years. This was touted in all the beauty magazines and by many bloggers. If you crave the deep cleansing which you can no longer get- consider a series of Dermasweep treatments. The treatment involves “sweeping” your face with a sterilized brush which varies in abrasiveness. This removes the dead skin layer and makes your complexion feel baby soft. Next, you can get infusions of up to 2 different serums. Questions I often am asked

1. What is the difference between Botox and a filler?

Botox (and the botulinum toxins Dysport, Xeomin, and Jeuveau) are muscle relaxants. They are injected into specific site on muscles, to prevent them from contracting. They are useful for eliminating wrinkles on the upper face that are due to muscle tensing. They relax the muscles between the eyebrows that causes frowning. Similarly they “iron out” wrinkles at the outer corner of the eyes and on the forehead when Botox is injected there. Treatment takes several days to appear and lasts 4 to 6 months.

Fillers are analogous to a “spackle”. They fill in lines and folds by adding volume. They work well when the lines are due to volume loss or are caused by gravity. We use products like the Juvederm and Restylane products to lift up a fold. The heavier products in these famiies can be used to augment cheekbones and “pull back” a sagging jaw, giving the appearance of a face lift …instantly and non invasively. Results lasts 6 to 12 months.

2. Do I need to wear a sunblock if my makeup has SPF?

People need different SPFs. If you are very fair or have had a skin cancer, the spf of 15 standard in most foundations will probably not be enough for you. It is better to apply at least an spf 30 daily under the foundation. If you are running, swimming, or playing sports then a higher spf should be applied and you can probably skip the foundation.

3. Do I need more than 1 sunblock?

I generally prefer a spray sunblock for the body. It can be easily applied and most are non greasy. You should use it on all skin not covered by clothing. Meaning- the neck, chest, arms, and legs.

I don’t like a spray for the face. It can accidentally enter the eyes, nose, ear, or mouth. Places it shouldn’t reach. For the face, try a product form a tube. It can be a lotion, cream, stick; or what ever feels good on your face. You know your skin type- if oily don’t use a cream. If you still break out with acne choose an oil-free option. Sticks are great for the nose, ears, and areas that sweat would rinse off a lesser sunblock.

Maskne- The new Acne

What is Maskne?

This word is a combination of mask and acne. Or, acne that forms underneath the masks we are all wearing now; covering our lower face. You may not have acne elsewhere. Generally there are small red bumps; but not comedones (blackheads), pustules, or cysts. It resembles perioral dermatitis, a rash around the mouth , and occasionally, around the eyes.

What causes maskne? A few things.

First, the heat and humidity stimulates our oil glands to produce more oil. This happens every summer, anyway.

Second, there may be friction from your mask. This can cause red bumps to appear. Generally there is no itching.

So, how does this condition get treated? Especially since we are going to be wearing masks for probably at least another few months?

Wash your face when it gets oily. You are probably using a restroom a few times daily. During one of these trips use the soap there to wash your face. Degreasing helps tremendously .
You may need topical medication for acne. Try Differin gel 0.1%, which is now over the counter. It is a mild retinoid and can help alleviate this condition. Use once or twice daily where needed only. If not enough help; then come see me for either a stronger prescription topical or else an oral medication for acne.

The New "Normal"

We are back!

We will all be wearing masks, and ask that you do too. We are concerned for your safety and are taking every precaution to distance patients . We are scheduling patients one per hour. Please do not come if you are feeling sick; we can schedule a tele-medicine visit for you instead. You can even send us a selfie for me to see your rash or skin growth. These visits are covered by most insurances, including Medicare for seniors.

We have hand sanitizer at the front desk. We also have hand washing stations. There is a debate over which is better, and I generally prefer hand washing whenever possible. I think of sanitizer for use when you are not near a sink, but many people are comforted by “science”. Hand sanitizers are alcohol based and tend to dry out your hands more than hand soap. To curb this problem, some companies have added moisturizing additives to their formula, such as aloe. Thankfully in summer the humidity in the air helps with this. Some individuals, especially those prone to allergies and eczema, may require a hand cream or even a prescription strength cortisone to heal their hands from the chopping of hand sanitizers and excessive hand washing.

Meet the New Barbie!

What’s new? I am happy to see that Mattel has released new Barbie dolls with Vitiligo and Alopecia. There is also a doll with a prosthetic limb and one with a wheelchair. These conditions are found in children and adults and show that people shouldn’t be ashamed of the conditions they have and can still be attractive and fashionable.

Vitiligo is an autoimmune disease where you develop chalky-white patches of skin. It is an autoimmune condition and we don't know the cause. It is often symmetrical, commonly affecting the face; but this condition can occur on any part of the body. There are a variety of treatments available to treat, including topical creams, lasers, ultraviolet therapy, etc. There are also concealing makeups to help hide the contrasting skin if this is preferred.

Alopecia (a specialty of mine) has many causes. The Barbie doll is bald on the scalp, reflecting alopecia totalis. This is a form of alopecia areata, that usually starts as coin-shaped areas of hair loss that can spread; or not. The entire body can become affected. There are many treatments for this condition. A popular office treatment is to have the spots injected with triamcinolone (a cortisone) on a monthly basis. Usually the hair regrows within several months. There are also topical therapies as well as the home laser helmet ( Theradome, which we have displayed in the waiting area) for treatment of the scalp.

What is PRP?

PRP stands for Platelet Rich Plasma. Platelets are a component of our blood and help with clotting and repair of tissues. Platelets contain numerous (10+) growth factors and help skin heal, such as from a cut, wound, etc. PRP treatments utilize your own blood to stimulate hair growth. They can also help improve acne scars when injected into them.

PRP has been used in sports medicine for many years to help athletes recover from injuries and return to play. In my Dermatology practice I use PRP on the scalp and on the face.

This treatment helps with all types of hair loss. PRP stimulates dormant and weak follicles to regain potency and grow thicker hair. It has been described as CPR for hair follicles. You can have just one treatment, but most people purchase a package of treatments (at a discount) since several sessions are usually necessary. You can continue topical medication, Theradome laser helmets (which also help grow scalp hair- and which we demo for you at the office), and oral hair loss medication during injections. For some people maintenance therapy is necessary.

The other use for Microneedling and PRP is to treat deep acne scars and melasma (facial pigmentation).

New Year's Resolutions

So, it’s a new year and let’s get off to a good start.

1. Wear sunscreen every day. Just on exposed skin. In New York, that generally means on your face, until the warm weather comes. There are a huge number of sunscreens available. During the winter it generally doesn’t have to be waterproof, and it will be less thick/ sticky. Our recommendation is the EltaMD SPF 46, a light-weight city sunblock, that even reduces redness. You can find it in our office. When you go on vacation you will need something for the body, and the EltaMD spray is my recommendation. The can makes it a snap to reach the back, arms, and legs. Waterproof and not at all greasy.

2. Make an appointment for a “body check” for skin cancer. There are ABCDE warning signs for moles. Any growth with Asymmetry, an uneven Border, multiple or a dark Color, Diameter larger than a pencil eraser, or a spot that is Evolving should be checked. The American Academy of Dermatology recommends a body exam for skin cancer annually. Should you spot something during the year, come in to have it evaluated/possibly biopsied, to that it doesn't grow.

What Are Crow's Feet And How Do I Prevent Them?

They say the eyes are the window to the soul, and they’re also a telltale sign of your living habits, primarily because the skin around your eyes is some of the thinnest and most delicate on the body. Having a hard time sleeping each night? Dark circles and bags will begin to form under your eyes. Find the joy in almost everything? You’ll have those pesky little laugh lines in no time. And don't get us wrong, laugh lines, or crow's feet, are a sign of a life well-lived. But if you would like to minimize their appearance, New York City dermatologist Joyce Davis breaks down everything you need to know.

What are crow's feet?

"Crow's feet are wrinkles at the outer corner of the eyes. They resemble the feet of a crow—hence the name," Davis explains. Before you start to freak out, crow's feet are normal. Here's the thing: As you age, the skin around your eye loses its elasticity and natural oils, making it more prone to wrinkles. "We develop crow's feet due to the loss of collagen and elastic tissue within our skin. This happens naturally, with aging. Free radical formation from sun and pollution can worsen the effects. We also see these beneath the eyes as a result of excessive smiling, and muscle contraction," she continued. "As we age, the lines get deeper and longer."

Is there a difference between fine lines and crow's feet?

"Fine lines are wrinkles that are not deep or wide," Davis said. "These can occur anywhere on the face." While crow's feet appear due to muscle contraction, fine lines aren't as deep and pronounced and can be treated differently than crow's feet. "Most fine lines can be eliminated with one of the lighter skin fillers." Davis recommends fillers like Juvederm Volbella, Resylane L, Restylane Refyne, or Belotero Balance.

What if you're not crazy about fillers?

Topical antioxidants can also offer significant results by eradicating the free radicals that degrade collagen and elastin. "I also recommend year-round use of sunglasses that filter UV light and a side bar that protects the outer eyes. Don't use mirrored lenses, which only attract more sunlight and bring more damage to your skin," she advises.

How do you prevent crow's feet?

First things first: Always wear sunscreen! "You want to start wearing SPF 30+ daily. And if you're near or in water or very active, make sure you're wearing a waterproof formula," she adds. You'll also want to start moisturizing eye creams containing proven ingredients like niacinamide, retinol and vitamin C. Ones with hyaluronic acid can also help plump skin and fill-in fine lines.

What is the best treatment for crow's feet?

Botox:

"My go-to treatment for crow's feet is a Botulinum Toxin (Botox), but Dysport, Jeuveau, and Xeomin all work well in this area, too," she explained. "Botox works by blocking the release of acetylcholine, a chemical needed for muscle contraction. I typically inject the outer corners of the eyes, but some patients also get a tiny amount injected in the muscle band below each eye. By stopping contraction, the creases and wrinkles temporarily disappear." Davis says the treatment can deliver natural results and last up to four months.

Fillers:

"Fillers can be used in the under-eye area to fill in hollows and lessen shadows. Only the lightest of fillers should be used here," she said. "My favorite is Volbella. Fortunately, it contains lidocaine, a local anesthetic, so treatment isn't too painful. Many of my patients come in 30 minutes earlier, anyway, to sit with a topical anesthetic cream to lessen pain further."

Why the rich look better?

Look at celebrities like Melania Trump. How does she look so good?

She has a glow! And not a wrinkle in sight.

To eliminate the “worried” or “angry” look celebrities use one of the toxins, like Botox. We have four different ones available for injection during a simple office visit. These will eliminate the frown line between the eyebrows and the horizontal forehead lines that make us mortals look old. Ditto for the outer corner of the eyes. Especially if you have had some sun.

Sunscreen controversies

What's the truth?

Several topics were in the news recently. One controversy is whether or not dark skinned individuals need to wear sunblock. It has been the policy of the American Academy of Dermatology to recommend sunblock, SPF 30+, year round, on all skin not covered by clothing. The low incidence of skin cancer in darker skinned individuals in sun exposed sites questions this. Bob Marley, the reggae super star died of melanoma, but it was on the nonpigmented skin of his toe. An area not sun exposed. The thinking now is that the melanin in dark skin may protect individuals from the sun naturally.

The other controversy is sunblock absorption. It does appear that sunblock gets absorbed . What this means is unclear and studies will need to resolve the importance of this.

Overall , the risk of melanoma is too great to ignore. Especially in young individuals. This is even more so in individuals who use tanning beds. Any way that we can prevent this potentially deadly disease is to be embraced. So, for now, keep using your sunblock and enjoy the rest of your summer !!

What's New?

We have a new botulinum toxin out …Jeuveau. It has all the features of Botox and perhaps lasts a little longer. Come in and try it to see for yourself.

What else is new? One of my patients came in asking for a filler for “stretched “ earlobes. She said that she saw the treatment on one of the housewives shows. It is a quick, easy treatment- and one syringe of Juvederm or Restylane should improve the lobe. Your earrings will hang better.

We have a new drug called Qbrexza (Really!) for treatment of excess localized sweating. It's a medicated sheet that you can wipe on your underarms, palms, soles, and other areas where you sweat a lot. Perfect timing for our hot NY summers!

I am quoted in the July 2019 issue of Good Housekeeping . Look for me on page 22, quoted in article on hair damage and care. Originally, the article was on split ends. They realized that by expanding the topic there was so much more they could discuss.

Remember to condition your hair. All the swimming we do in the ocean and pools, dunking in hot tubs will dry our hair. Put some conditioner on when you come out. At least on the ends to prevent damage.

May is skin cancer awareness month

Come in for a full body skin cancer screening !

Some facts :

1.Any skin spot that is crusting, bleeding, and not healing should be checked for skin cancer.

2.Any skin lesion that is irregular in size, shape, and/or color should be checked. Especially if there is black in the lesion and/or it is changing.

3.Sunblock recommendations:

* Wear sunblock year round on all skin not covered by clothing.

* Use a light weight, comfortable block for the city; but a heavier, water resistant one for sports and when in or near water.

* Throw out old sunblock. All sunblock is labelled with an expiration date. If yours has none, it is very old and should immediately be discarded. If your sunblock smells, is separating, or turning color, throw it out.

* Keep a variety of sunblocks on hand. A spray is helpful for reaching your back and body. A lotion is ideal for the face. A stick is great for the nose, lips, and ears.

Spring Time Dermatology

Spring is an in between season. Winter’s cold, dry weather has taken a beating on us and summer humidity hasn’t yet made an appearance.

Priorities are to get ready to bare some skin ! Soon you won’t be able to hide under black tights, pants, and thick sweaters. All your skin problems will show. The good news is that we have excellent treatments for all your needs.

Get your legs ready for shorts and swimsuits. Sclerotherapy is a non-surgical, injection therapy that will eliminate spider veins on your legs. We use Asclera, a newly FDA approved, virtually painless medication that has been used in Europe for decades. Some veins will be eliminated with just one session. Others may require two to three sessions. Expect some temporary bruising and then beautiful legs! The spring is the right time to begin treatment. Keratosis Pilaris, or “chicken skin” is a common skin condition on the upper arms. Occasionally it appears on the thighs, back, and face. Is diet important for “good” skin?

Dermatologists used to think so. When I was in high school and college (and suffering from acne) I was told to avoid chocolate, nuts, cola beverages, and most of the teenage diet. At some point my father said there was no point since I was still “breaking out”. I went to a number of wonderful dermatologists and they preached what was believed correct at that time.

Flash forward to medical school and the news that diet makes no difference for acne! I probably gained 10 pounds that first year, trying all different types of chocolate bars.

Most dermatologists now realize that acne results from elevated male hormone levels (in both men and women) stimulating oil glands. That’s why acne increases with stress; during exams, romantic breakups, puberty, etc. Oil gland secretion also increases in warmer weather. Too much oil on the skin clogs pores leading to blackheads, pustules, and cysts. That’s why I’m a stickler for reading labels and avoiding products with oil.

But is the pendulum swinging back? Articles in dermatology journals frequently refer to a high “glycemic “diet as contributing to acne. They claim that this works by triggering hormones that stimulate production of acne. The jury is out. We will see!

The stuff I love

In the midst of a cold New York winter we have Valentine's Day; a day of romance and bringing out your sexiest self.

But this takes preparation and taking care of your skin before the "big day". So, what do I use ?? My favorite serum is the High Potency Vit C Brightening Serum that we dispense. I apply this directly to my face each morning. It takes out redness and helps to stimulate collagen formation. It also gives me a glow, which is always fun. At night I apply a retinol cream, since the stronger retinoids make my face too dry and irritated. I like our TIZO PM Restore, since it contains antioxidants and ceramides to lessen dryness. For moisturizing my hands I keep Aveeno Daily moisturizing lotion all over my apartment; by the kitchen sink, on my nightstand, and in my bathroom. My kids like it too.

For foundation (liquid makeup) I use Prescriptives Flawless Skin Makeup with SPF 15 that I was introduced to by a makeup artist before my wedding 20+ years ago. I switch to their lighter Virtual Skin foundation in the summer months. I always apply it with a wedge sponge that I change once or twice a week. Prescriptives is no longer selling their makeup in department stores, but I haven't found the on line ordering to be a disadvantage since they have a good return policy if the color isn't right. They have also guided me on the phone when I needed my color adjusted. Complexions change as we age. We get paler, and my makeup was looking too dark. They suggested a lighter color in the same family and it works. For blush I have always favored a powder and like Nars Orgasm. It looks great on everyone and I have even given it as gifts.

Of course, we do have plenty of "quick fixes" if you need some immediate improvement.....

New Year's Resolutions

Include a better version of yourself in your resolutions!

This is the time to start taking care of yourself, and that includes your skin. Resolution #1 to make is coming in for an annual mole check for skin cancer. This is especially important if you are fair, have a history of skin cancer, and/or lived in a sunny climate. Any spot that is bleeding, crusting, or changing color should be examined. Anything black should also be examined.

Resolution #2 is to wear sunblock daily. Only skin not covered by clothing need be protected. Don’t use any product with SPF less than 30; it will be less effective. I recommend a lotion for the face and a spray for the body. This is an easy, nongreasy way to cover the skin on the arms/legs/chest when they are exposed.

Resolution #3 is to take an interest in your appearance. We have many nonsurgical tricks for tightening the skin, lifting the skin, and minimizing wrinkles and folds. Look younger in half an hour. Maintain your results with all- in- one antiaging/SPF products. Now is also a great time to begin laser hair removal. (see our promos for a discount on individual treatments and packages). It takes about six months of treatments to eliminate hair so begin now and have silky smooth legs, arms, underarms, and the bikini in time for summer ! The same is true for eliminating leg veins. Begin treatment now so that any temporary bruising will be hidden by dark clothing.

The "Non-Surgical Facelift"

I'm sure you've heard of it, but ever wonder what it is exactly?

We hear the term "non-surgical " facelift used often. But, what is this procedure? It basically refers to tightening the skin on the face without cutting the skin.

I have been employing this technique for years. I can use a variety of heavier/ stronger fillers to tighten the jawline and lift up the cheeks. You will look rested and about 10 years younger in just half an hour. Sessions need to be repeated every 9 to 12 months for maintenance.

There are some newer and exciting fillers in the Juvederm family. Voluma is used to lift the apples of the cheeks and add volume. There is also Vollure, a longer acting filler similar to Juvederm. The lighter Volbella is good for fine lines above the lip and for filling in under eye hollows. We also still offer regular Juvederm (ultra) and the more robust Juvederm ultra plus.

So, the weather is getting cold !! We have some special products for easing your skin into the drier, cooler weather. Loss of the summer humidity means that your skin will dry out. I have been having fun working with Clairol as a hair and scalp expert on their new Nice'n Easy formula. This is a powerful advance. Transition to Fall Skin/ body treatments

Changing your routine is vital for beautiful looking skin this fall!

With the end of summer and start of the fall, humidity drops and skin starts to suffer. At this time you may need to switch to a more moisturizing soap and resume use of a moisturizer right after bathing (to "lock in" water). If you have been using a lotion it is time to switch to a heavier cream. Look for ceramides in the ingredients. These restore the natural lipid component that should be in your skin, but decreases with aging. The EltaMD moisture- rich body creme (sold in physician offices like ours) is an ideal product for skin hydration. The Aveeno daily moisturizing lotion is also helpful and widely avaiable. Both are non greasy.

September is also an ideal time to begin "work" on the body, since healing can be hidden by clothing. leg veins can be eliminated with injection therapy and brown spots of the trunk can easily be removed with liquid nitrogen freezing. Laser hair removal should also begin at this time so you won't have to shave next summer!

September is also an ideal time to get rid of face discoloration from too much summer sun. Come in for a laser treatment of your face or a chemical peel to even out your skin tone and refresh your complexion.

Poison Ivy

Poison Ivy contains an oil (urushiol), which causes an itchy rash to form on contact with the plant. Over 90% of the population is allergic. You can get linear blisters from touching any part of the plant, or incredible itching from contact with items that have touched the plant and still have the oil on them (like your dog). Even burning the plant can cause an allergic reaction to form on the face from aerosolized oils.

What to do? If you have been hiking or gardening in areas where the plant grows, immediately shower when coming inside. Remove your clothes for washing, and include washing all items that have touched the plant (sunglasses, gardening tools, hats, sneakers, jewelry, etc.). They will hold the oils and re-inoculate you if not cleaned.

If your skin was touched by the plan you will probably get a rash, which will need prescription strength topical cortisone to treat. If the rash is widespread, then oral cortisone (prednisone) may be necessary.

Last, make sure that items like your dog get bathed; also slipcovers if you sat on them before showering; so that you don’t keep reintroducing the oil to your skin.

Best way to stay protected this summer

During the summer months we all look to spend more time outdoors. But, with all the concern about mosquito and tick borne diseases what is the best way to protect out skin from bites? I was just interviewed for the Huffington Post
on this subject. I had the chance to review material on all the natural insect repellents (and found out that Oil of Citronella doesn't work as well as we all thought), and that DEET is really the gold standard , and can be used safely.

Skin cancer Awareness

Skin cancer is common and almost always curable when detected early. There are 3 main types of skin cancer:

Basal Cell Carcinoma - This type of cancer most often occurs on sun exposed sites in fair skinned individuals- but that is changing. Darker skinned individuals and even Black individuals can get this type of skin cancer. It first appears as a pearly “bump”, which may crust and bleed from time to time. It can even “fall off” and recur. In darker skinned individuals it often has streaks of brown or black.

Squamous Cell Carcinoma - Usually appears as a crusted lesion, which doesn’t heal. If the crust should fall off it will grow back at the same site. Again, these lesions are most often found on the head and neck, where there is maximum sun exposure. They are especially common on the scalp of men with thinning hair.

Melanoma - This is the most serious form of skin cancer. Unlike Basal and Squamous Cell Carcinoma, which grow slowly and locally; melanoma can spread into the lymph nodes and travel to distant organs. I encourage everyone to get a full body skin exam once yearly so that these lesions are discovered at the earliest opportunity. If you notice a spot which appears dark, new, and irregular in size, shape, and color- call me to have it examined and possibly biopsied. Early discovery and treatment can literally be lifesaving!

Make sure that you avoid tanning beds which are known to increase your risk of skin cancer exponentially!

Seems that Spring may have just started this MAY! Start thinking about wearing sunblock regularly... if you haven't started yet!

This year it seems like MAY is the start of springtime in New York. Weather gets warmer and we spend more time outdoors. Now is a good time to address sun protection, which should be worn daily on all exposed skin.

Sunblocks come in many forms and strengths; use something with at least SPF 30. An SPF stick is useful for the lips and nose. Not only will it help guard against sun damage, skin cancer and aging; but daily use on the lips will help prevent reactivation of cold sores.

SPF sprays are easy for achieving a quick, uniform, non-greasy application to the arms, legs, chest, and back. Most are waterproof and many are oil free (good for acne prone skin). SPF sprays also work well for protecting the scalp, especially when hair has thinned. Lotions are best for the face. Make sure it is oil free if you are acne prone. Find one that is comfortable to wear and use instead of a moisturizer.

Hats (with a wide brim- not baseball caps) are helpful for shading the face and sunglasses are also useful. Avoid mirrored lenses, since these will reflect additional sunlight onto your face.

Laser Hair Removal Season is here!

Winter is behind us and we will be skipping pantyhose. Laser hair removal is the way to get your legs silky smooth in time for summer. Our light sheet duet laser is pain free and fast! No messy gels are needed with this machine and a series of 6 treatments will get your legs summer ready. Come in on your lunch break!

To get the best results from your treatment keep the following in mind:

Shave your hair the morning of your treatment; or the night before if you can’t shave in the morning.
Don’t wax your hair between laser hair removal treatments. Waxing removes hair by the follicle and the laser needs a follicle to be present (to destroy it and stop if from producing hair). So, if you don’t have a follicle to treat, you are wasting your laser session.
Get your treatments before your hairs turn white. Lasers don’t work on hair that has lost its pigment.
Don’t come in tan. Even using a self- tanner is not a good idea.
Don’t apply lotions or oils the day of your treatment. It can interfere with the laser.

Chopped Lips

Chopped Lips? Fix them in time for Valentine's Day!

With cold February weather upon us, our lips need attention too. Lips should be kept moisturized or else they will crack! The medical term is cheilitis, which means try, cracked, and peeling lips. Lip licking, done to moisturize the skin, and common in children, is especially bad and actually makes the problem worse. Your saliva will evaporate and leave your lips drier than before. Picking or biting off dried skin is also bad and will result in open sores which are painful.

So what now? Women can switch to a more moisturizing lipstick during these months. Use of a tinted chapstick is also and easy remedy. Most of these also contain sunblock, which is great! Men and children should stick to untinted lip balms (flavoring is available to increase compliance). look for one with SPF for maximum benefits. This is important for runners, people who ski, ice skate, go sledding, play in the snow, ot otherwise spend time outdoors. There are many brands to choose from; available in sticks, tubes, pots or tins. Find one you like and make sure you carry it with you.

Taking Care of Your Skin in the Cold Weather

Our skin changes with the weather. During our warm, humid NYC summers our skin fills with moisture and lotions are hardly needed. In the winter, NYC air becomes dry and our skin does too. You may see dry patches and cracks forming, especially where skin is washed often, such as on our hands.

What to do?

First, wear gloves! When going outdoors winter gloves should be worn. Keeping the wind and cold air away from our hands is essential. When cleaning dishes or doing housework with detergents rubber gloves should be worn, since our hands are extra susceptible to chapping from harsh cleansers at this time.

Second, start using a moisturizer. Apply it after washing your hands. Start with a lotion. There are many drug store brands available to try. My favorite is the Aveeno daily moisturizing lotion, with oatmeal to soothe itching. You can test the EltaMD moisture rich body cream, at our office when you stop in. Creams are heavier than lotions and many people prefer these in the winter because they adhere better.

If eczema flares because it is triggered by the harsh winter, you may need a prescription-strength corticosteroid medication or Eucrisa, the new non-corticosteroid medication that you may have seen advertised on television. It works well and is even safe for use in children.
